Youth Development & Kidney Health Transition Worker

University Hospital Southampton NHS Foundation Trust is seeking an experienced youth worker to support young people with a long-term kidney condition. The role involves online programmes, emotional wellbeing groups and community projects, with intensive 1-to-1 interventions either in person or virtually.

You will work within a multidisciplinary team and commonly cover evenings and some weekend work. The post is part-time and hybrid, collaborating with another renal youth worker.
