Job Description:
Essex Partnership NHS Foundation Trust is seeking a Social Worker to assess needs and develop, implement and evaluate care plans for Adults and Older Adults in the Crisis Resolution Home Treatment Team. The post reports to the Clinical Lead and works with multidisciplinary teams across wards and primary care.
It requires completing a preceptorship and delivering high-quality, person-centred care within an integrated service.
