Forensic Care Coordinator – Community Psychiatric Nurse

South West London & St George’s Mental Health NHS Trust is recruiting a Forensic Community Psychiatric Nurse and Care Co-ordinator to manage a defined caseload of adults with forensic histories. You will act as care co-ordinator and social supervisor within an MDT, providing risk assessment, engagement, and recovery-focused interventions.

The role includes collaboration with police, MOJ, probation, housing and social care, with regular supervision and professional development opportunities.
