New Start is seeking an experienced Aftercare & Relapse Prevention Practitioner to join its Adult Services division in Liverpool. You will support the Step-down Service and Harmony Project, advising residents on recovery, tenancy sustainment and social reintegration.
The role involves crisis support, risk assessment, recording tasks and working within health and safety guidelines to help residents stay housed and engaged in meaningful activities.
