Senior Community Nurse – Learning Disability MDT Lead

Tees Esk and Wear Valleys NHS Foundation Trust in York and Selby invites applications for a Band 6 Senior Registered Nurse in the Community Learning Disability team. The role involves working within a Multi-Disciplinary Team to support adults with learning disabilities in the community, delivering assessments, care planning and interventions.
