Senior Advanced Clinical Practitioner – Home Visits Lead

Derbyshire Community Health Services NHS Foundation Trust is seeking an Advanced Clinical Practitioner to support a core team of ACPs across the PCN North Hardwick and Bolsover with home visits for seven practices.
