Discharge Care Social Worker – Home First Team (7-day)

The City of Doncaster Council is seeking Social Workers for the Integrated Discharge Team in the Home First Directorate. Positions include one full-time and one 4 days-per-week role, based at the civic office, covering post-discharge assessments and planning for adults.

A 7-day service with occasional weekends is included. Applicants should have experience with the Care Act 2014, Mental Capacity Act 2005, Hospital Discharge policy 2020, GDPR, and safeguarding.
