Job Description:
Forward Leeds is seeking a Community Alcohol Worker to provide flexible, community-based support for adults with high-risk alcohol dependence. You will manage a caseload, deliver person-centred interventions and coordinate care with partner services.
Based across Leeds, you will conduct home visits, work with GP surgeries and hospitals, and support detoxification and rehabilitation pathways while upholding safeguarding and equality.
