Location: Melton Mowbray
Job Description:

Flexible Working | Permanent Role Support | Excellent Benefits

Are you looking for a Post-Discharge Social Work role where you can make a real difference, develop your skills, and feel genuinely supported?

We’re recruiting for an exciting permanent opportunity within a busy Transfer of Care / Hospital Discharge Team, helping people settle safely back into the community and regain their independence following a hospital stay.

At Leaders in Care, we understand the dedication and challenges that come with a career in social work. That’s why we work closely with onboarding teams to support a smooth transition into a secure permanent position, helping you feel confident and supported from day one.

This role offers an excellent benefits package, including hybrid working with the opportunity to work from home up to two days per week.

About the Role

Supporting a busy 200-bed discharge pathway, your focus will be on assessing people’s ongoing needs after discharge, reviewing care arrangements, and ensuring they receive the right support to maximise their independence.

Your responsibilities will include:

  • Completing post-discharge assessments within a 14–28 day timeframe
  • Carrying out 6–12 week reviews to assess progress and adapt support where needed
  • Using a light-touch triage approach to identify the appropriate level of intervention
  • Reviewing care packages and coordinating further support or re-referrals where required
  • Working collaboratively with health professionals and partner agencies to promote safe, sustainable outcomes
  • Promoting independence and helping reduce avoidable hospital readmissions

You’ll join a supportive, multi-disciplinary team where collaboration, communication, and person-centred practice are at the heart of the service.

What You Can Expect

A positive and inclusive team culture where your ideas are valued, your professional development is encouraged, and your wellbeing is prioritised.

Please note: As this is a statutory Social Work position, you must hold a recognised Social Work qualification and be registered with Social Work England
