Hospital Discharge Social Worker – Promote Independence

Connect2Halton on behalf of Halton Borough Council is seeking an experienced Social Worker to join the Hospital Discharge Team at Whiston Hospital in Prescot. You will conduct strengths-based assessments and develop personalised support plans to support timely discharges, working with adults 18+ with diverse needs.

The role requires knowledge of the Care Act 2014 and Mental Capacity Act 2005, the ability to work with multidisciplinary teams, manage your caseload, and advocate for independence
