Job Description:
Mersey Care NHS Foundation Trust is recruiting a Band 6 Mental Health Transition Practitioner to support 18–25s transitioning to adult CMHT/Recovery teams. You will work with CYP services, carers and referrers, conducting biopsychosocial assessments and linking young adults to appropriate pathways.
You will be part of the CMHT MDT, promoting transition principles and working across the wider system to ensure a seamless, person-centred service.
