Westbury-on-Trym Practice in Bristol is seeking a friendly Care Navigator to join our reception and care navigation team for a fixed-term maternity cover of approximately 6 months. You will help patients access appropriate care, manage appointments, and signpost to relevant services.
The role involves covering the care navigator duties during sickness/annual leave, taking messages, and coordinating with healthcare professionals. 14.5 hours per week, with scope for flexible additional hours.
