Prison-Based Recovery Specialist

The Forward Trust in Swansea is seeking a compassionate Recovery Practitioner to support prisoners aged 18 and over within an integrated healthcare framework. You will manage your own caseload, deliver assessments, SMART care plans and evidence-based interventions designed to help clients move forward with their recovery journey.

You will collaborate with healthcare, probation and partner agencies, provide group sessions and maintain accurate records, contributing to service targets and outcomes
