Senior Nurse, Virtual Ward – Home-Based Care & Innovation

Nottinghamshire Healthcare NHS Foundation Trust is seeking a Senior Community Practitioner (Nurses) to join the Nottinghamshire Virtual Ward. The role is full time and focuses on delivering hospital-level care at home, including advanced assessment and decision-making in patients’ homes within a Band 6 framework.

You will work autonomously and within an MDT, support safe discharge, and develop skills in remote monitoring and digital care, contributing to system transformation and patient safety.
