Pediatric PATCH Nurse: Home-Based Acute Care

Chelsea and Westminster Hospital NHS Foundation Trust is seeking a post holder who will use their clinical experience to independently assess and manage acutely unwell children, ensuring all children referred to the PATCH team receive high quality, compassionate and effective care at home.

The role involves face-to-face, virtual and telephone consultations, and where escalation is required, liaison with hospital teams to maintain timely care.
