Social Care Assessor – Enable Independence & Wellbeing

Isle of Anglesey County Council is seeking a Social Care Assessor to work across Social Care, all ages, within a multidisciplinary, multi-agency framework on Anglesey. You will manage an allocated caseload, undertake needs assessments, develop care and support plans, and coordinate with Health and other partners to deliver improved outcomes for adults, children and carers.

The role supports flexible deployment to other teams to develop knowledge and skills, and may require Welsh language skills
