Westminster City Council is seeking a Social Worker for the Hospital Discharge Team. The role, based at St Mary’s Hospital and across Westminster, involves assessments, safeguarding, and collaboration with health colleagues to support discharge planning.
You will work five days a week, including weekends, with a Duty Manager support arrangement. Requires CQSW/Degree in Social Work and registration with Social Work England.
