End-of-Life Community Support Worker — Home-Based Care

Cornwall Partnership NHS Foundation Trust is expanding its Community and End of Life Care services to deliver high-quality, patient-centred care in patients’ homes across Cornwall and the Isles of Scilly.

The Community Support Worker will work within the End of Life Care service to support patients with complex needs, collaborate with health and social care teams, and help reduce hospital admissions while enabling care at home.
