Lewisham and Greenwich NHS Trust is seeking a Clinical Primary Care Pharmacist (LIMOS) to join the interface and community medicines optimisation service. You will conduct medication reviews, provide expert medicines advice, and work across hospital and community settings to improve safe, effective prescribing for older patients with polypharmacy.
The role involves collaboration with MDTs, delivering training, and supporting pathways in Lewisham and the surrounding community as part of LIMOS.
