Youth Wellbeing & Social Prescribing Coordinator

The Blandford Group Practice is seeking a Young Persons Care Coordinator to join our Social Prescribing Team. You will support young people aged 5–25 and their families, signpost to services, and help improve wellbeing through personalised plans and community activities.

We value safeguarding, effective communication and the ability to build trusting relationships. The role requires flexibility for occasional evenings or weekends and a full UK driving license.
