Senior Transfer of Care Practitioner – Community-Based

EAST LONDON NHS FOUNDATION TRUST is seeking a Transfer of Care coordinator to manage a designated number of Transfer of Care patients in the community, coordinating initial assessments, developing care plans and onward referrals.

You will work autonomously using your own vehicle, support a multidisciplinary team across homes, clinics and residential settings, and participate in in-reach work into Bedfordshire Hospitals.
