Placements & Care Coordination Specialist

Pharmacyupdate is seeking a dedicated Care Coordinator to ensure each young person’s care is planned, delivered, recorded and reviewed in line with their care plan and regulatory requirements. You will be the main point of coordination between the placement team, the Placement Manager and external professionals.

You will coordinate day-to-day placements, monitor staffing and safety checks, support multi‑agency meetings and maintain accurate records in line with CQC and GDPR policies.
