Care Continuity Coordinator – Prison Health & Transfers

Practice Plus Group is recruiting a Transfer and Discharge Coordinator for the Substance Misuse team at HMP Lewes in Lewes. The role is a fixed-term contract for 12 months, Monday to Friday, 08:00–16:00, based on site.

You will support safe transfers, discharge planning, and continuity of care, with responsibilities across hospital and community settings while coordinating with the recovery team and ensuring patient needs are met during transfers.
