Team Manager (IDT)

Overview

As Team Manager in the Integrated Discharge Team, you will oversee social care operations across two hospital sites, ensuring safe, timely discharges and high-quality, autonomous practice. You will lead multi-disciplinary collaboration with health colleagues and manage performance and budget for your team. You will report to the Interim Head of IDT and contribute to ongoing service improvements within a health-led environment. This role supports the One Rotherham vision and the transformation of adult social care to maximise independence and wellbeing.

Responsibilities

• Oversee day-to-day social care management and supervision for the IDT
• Support and safeguard autonomous, person-centred practice
• Foster effective multi-disciplinary collaboration with health colleagues and challenge where needed
• Manage team performance and budget in partnership with other Team Managers
• Ensure discharge pathways meet needs and maintain high standards of care
• Coordinate across two hospital sites and align with organisational standards and service improvements
• Report to the Interim Head of IDT and contribute to continuous improvement initiatives

Key requirements

• Passionate about working with adults 18+ and delivering strengths-based outcomes
• Solid understanding of the Care Act 2014 and Mental Capacity Act 2005 including safeguarding duties
• Willingness to work flexibly and be on-site at least three days per week at both hospital sites
• Extensive experience of multi-disciplinary working
• Extensive safeguarding experience
