Support Coordinator: Lead a Person-Centered Care Team

Creative Support in Warrington is seeking a dedicated Support Coordinator to lead a team of care staff in our Warrington Services. The role involves delivering direct, person-centred care in tenants’ homes, coordinating service delivery and promoting strong partnerships within the community.

The successful candidate will have a proven management background, be organised and flexible, and participate in on-call duties as part of a 37.5-hour week on a 24-hour rota.
