Compassionate Hospice Social Worker — Flexible Scheduling

Patriot Healthcare seeks a Hospice Social Worker to manage the social service needs of patients and families within a hospice setting. You will provide emotional support, conduct needs assessments, and coordinate discharge planning as part of an interdisciplinary team.

The role requires a Master’s or Bachelor’s in social work, CSWE accreditation, and state licensure (LBSW/LMSW/LISW-CP) with at least one year of health-care social service experience; hospice experience preferred.
