Senior Community Occupational Therapist – Home-Based Rehab

The Calderdale and Huddersfield NHS Foundation Trust is seeking an experienced Band 6 Occupational Therapist to join the Calderdale Community Rehabilitation Team. The role involves delivering patient-centred rehabilitation with assessments, home visits and collaborative goal setting.

Most clinical work occurs in patients’ homes (including care homes), with clinics or group sessions as appropriate, and occasional hospital visits to support meetings and complex discharge planning.
