Adulthood Prep Social Worker for Youth with Disabilities

Dorset Council is offering a fixed-term full-time Social Worker role within the Preparation for Adulthood Team in Children’s Services. You will work with young people aged 14+ and families to develop independence plans, offering information, guidance and support.

The role may involve adult Care Act assessments and other related duties, with travel as required. You will join a multidisciplinary team including adults, children’s social care, education and SEN colleagues.
