Support Coordinator — Empowering Independent Living

Creative Support’s Doncaster-based Supported Living Service is seeking a caring, dynamic Support Coordinator to coordinate support for 28-30 individuals across 3 locations. You will work with families, external professionals, and staff to ensure high-quality, person-centred care that promotes independence and positive outcomes.

We require a minimum of 3 years’ experience with learning disabilities, experience in needs assessments, support planning and risk assessments, and a clean UK driving
