Senior Social Worker – Discharge & Liaison (Older Adults)

Portsmouth City Council is recruiting a Senior Social Worker to join the Older Person Mental Health Team in Portsmouth. You will lead discharge planning and liaison between NHS Trusts, inpatient wards and community services to ensure safe, timely transitions of care for older adults.

You will assess risk, coordinate with families and staff, and apply strength-based practice while navigating statutory guidance to deliver person-centred care across health and social care settings.
