Community Support Worker – Enable Independence & Wellbeing

Leeds City Council is seeking a dedicated Support Worker to assist adults with dementia or physical impairments within Day Opportunities at Holt Park Active. The role focuses on delivering person-centred support, promoting independence and community engagement, and working across services with flexible hours and a 30-hour week.

You will contribute to risk assessments, care plans, and signpost to relevant services while maintaining safeguarding and confidentiality.
