Marie Curie is recruiting for a Clinical Nurse Specialist in Sunderland Identification and Support Service. The role focuses on identifying and supporting people approaching the last year of life, using the NHS Early Identification Tool and coordinating care with GP practices and community teams.
You will work within Primary Care, conduct home visits, provide expert advice, and support advance care planning, while managing a caseload and contributing to training and service development.
