Role Overview
The SPA Clinical Triage Nurse receives referrals from a diverse range of people and services, triages them, and ensures they reach the appropriate service quickly. The role monitors the Health Call system used by heart failure patients, escalating out‑of‑range observations. It also includes Palcall, a telephone support and signposting service for people within their last 12 months of life. This is a 24/7 service.
Responsibilities
• Receive and triage referrals to appropriate services.• Monitor the Health Call system for heart failure patients and ensure timely action on out‑of‑range observations.• Provide telephone support and signposting through Palcall for patients in their final year of life.• Work across North Tees and South Tees sites as part of horizontal integration, maintaining knowledge of health care records for both localities.• Adapt to evolving service delivery and respond promptly to changing needs across a large geographical region.
Person Specification
Qualifications
- Registered Nurse (RN)
- At least 2 years post‑registration experience
- Community experience preferred

Salary: £39,959 to £48,117 per annum pro‑rate.
