Mental Health Outreach Support Worker: Person-Centered Care

Westminster City Council is seeking a Mental Health Support Worker to join the Mental Health Outreach Service in London. The role focuses on identifying needs, developing tailored care plans, and building respectful relationships to promote independence and wellbeing.

You will assess, plan, implement, and evaluate activities within a person-centred framework and produce reports to demonstrate impact. DBS enhanced check is required and the role is permanent with 36 hours per week.
