Autonomous Community Nurse – Home Care (Evenings/Weekends)

Cornwall Partnership NHS Foundation Trust invites applications for a Community Nurse in the Toressa Community Nursing Team (Band 5). This permanent role offers 37.5 hours per week across evenings, weekends and bank holidays, with options for full-time or part-time working from Penntorr Health Centre, St Barnabas Hospital, Torpoint and Saltash.

You will assess, plan, implement and evaluate care in patients’ homes, working autonomously yet in partnership with carers and MDT colleagues.
