SEND Transitions Coordinator — Inclusive College Pathways

DN Colleges Group is recruiting an Inclusive Transition Coordinator to join our Student Services/SEND team in Scunthorpe. You will support learners with SEND as they transition into college, coordinating personalised plans and liaising with schools, families, curriculum teams and external agencies.

This rewarding role focuses on inclusion, independence and ensuring every learner can progress. You will manage a caseload, track progress and contribute to EHCPs and transition planning.
