Senior Palliative Care CNS – Community-Based Specialist Nurse

City Health Care Partnership CIC is seeking a Macmillan Clinical Nurse Specialist in palliative care to provide expert advice, assessment and support for patients, families and clinicians, with a strong focus on community-based care designed to keep people out of hospital.

You will lead high-quality, person-centred care, work independently and with multidisciplinary teams, and contribute to developing palliative care services across the organisation.
