Description

We are currently seeking a Band 6 Case Coordinator to work with our NHS client within the Restormel team in Cornwall. This role involves managing a caseload of patients in the community and requires a full driving licence, as travel between patients may be necessary.

The role is Monday to Friday, 09:00–17:00. Mileage can be claimed for work-related travel. Please note that the Trust is unable to provide accommodation for this booking. This Role also falls inside IR35.

Job Role

• Coordinate and manage a caseload of patients within the community.
• Support patients in line with their individual care needs.
• Work collaboratively with relevant healthcare professionals and community services.
• Maintain appropriate patient records and documentation.
• Travel within the local community as required to support patient care.
• Contribute to effective communication and continuity of care across the team.
• Essential for the role

Relevant Experience

• Relevant experience in case coordination or community healthcare.
• Full, valid driving licence.
• Ability to manage a community caseload effectively.
• Strong communication and organisational skills.
• Ability to work independently and as part of a multidisciplinary team.
• Availability to work Monday to Friday, 09:00–17:00.
