Regional Home Support Coordinator – Wiltshire

Beyond Dementia is seeking a Home Support Regional Care Coordinator to oversee the Home Support Service across North and South Wiltshire, coordinating staffing, referrals and service delivery to ensure continuity of care. You will be a central contact for support workers, service users and carers, helping deliver flexible, person-centred support across our sites.

The role requires collaboration within a team and may involve attending or working from other sites as needed.
