Primary Care Pharmacist – Prescribing Pathway

Operose Health is recruiting a pharmacist to support local primary care delivery in London. The role focuses on safe prescribing, triage, and coordinating care with clinicians to improve patient experience and outcomes.

You will run triage clinics, review hospital communications, and manage repeat prescriptions while participating in home visits and immunisation initiatives. GPhC registration and teamwork are essential.
