Senior Community Nurse (Band 6) – Discharge to Assess

Stockport NHS Foundation Trust is seeking a Registered Nurse (Band 6) to join the Discharge to Assess/Intermediate Care at Home Team in the community across Stockport. The role is permanent, 37.5 hours, and requires flexible working across organisational boundaries to support discharge planning.

The service provides multi-professional assessments on discharge from hospital and delivers holistic nursing and rehabilitation in patients’ own homes, with 7 days a week coverage 08:00–21:00.
