We’re looking for someone with excellent communication and organisational skills who can remain accurate and professional while working in a busy environment.This is an ideal opportunity for someone who is organised, confident using digital systems and enjoys working with people in a busy healthcare environment. The Care Navigator will help ensure patients receive an efficient, supportive and seamless experience when accessing our services. They will be responsible for receiving, reviewing and processing patient requests, ensuring information is accurately recorded and directed to the appropriate clinical or administrative pathway. The Care Navigator will also help patients access the right care by signposting them to appropriate practice, community, external or self-referral services. Full role-specific training will be provided and may include clinical and digital patient access systems, care navigation pathways, red-flag awareness, clinical escalation procedures, patient signposting
safeguarding and Information Governance.
