Countess of Chester Hospital NHS Foundation Trust is seeking a Rehab Assistant to join the Chester Community Response Hub, providing early supported discharge services to patients from The Countess of Chester Hospital and Ellesmere Port Hospital. The role requires a full driving licence and car, and shifts across 7 days between 8am and 9pm.
You will work with therapists and nurses, support patients at home per Care Plan, and contribute to safe, independent living.
