Job Description:
Western Health & Social Care Trust is recruiting an Advanced Practitioner Occupational Therapist to lead the Frailty Service within Acute Inpatient Services. You will deliver high-quality assessments, develop care plans and coordinate discharge arrangements for older adults, working within a multidisciplinary team.
The post is at Band 7 level and includes flexible working options from day one, with a focus on improving patient outcomes in acute settings across the Trust.
