Herefordshire and Worcestershire Health and Care NHS Trust is seeking an experienced Occupational Therapist to join the Urgent Community Response team in the Worcestershire SPA, Worcester. You will triage referrals, work with Neighbourhood Teams, and support end-of-life pathways across multiple sites.
Applicants must hold HCPC registration and a degree/diploma in Occupational Therapy, with a record of CPD. Flexible hours and MDT collaboration are required.
