Job Description:
Torbay and South Devon NHS Foundation Trust is recruiting a clinical pharmacist for the Harbour Service, delivering integrated frailty care in the community and in hospital settings, including Newton Abbot Hospital. You will visit patients, review those using the Harbour at Home service, participate in the SDEC unit, and support ward-based care with comprehensive medication reviews.
Non-medical prescribing qualifications would be welcomed and developed within your scope.
