Mid and South Essex NHS Foundation Trust in Chelmsford is seeking an experienced IDT Nurse Assessor (Band 6) for maternity leave cover until August 2027. The role focuses on discharging patients safely from hospital to home or onward care, coordinating assessments and referrals with community partners.
You’ll work across multidisciplinary teams, supporting patients with complex needs, ensuring timely, person-centred discharge planning and smooth transfers of care within Broomfield Hospital.
