Community Therapy Assistant — Independent Home Rehab

County Durham & Darlington NHS Foundation Trust is seeking a Therapy Assistant to join the Community Therapy North Team at Chester Le Street Community Hospital, working with Physiotherapists, Occupational Therapists and Associate Practitioners in community and hospital settings.

The role includes independent and joint visits across North Durham, documentation at point of care, lone worker requirements, and participation in professional development and training.
