Hospice at Home Specialist Nurse — Lead & Care Team

Longfield Hospice Care in Nr Stroud is seeking a Specialist Nurse – Hospice at Home to join our hospice at home team. You will manage a community-based caseload of patients requiring palliative and end-of-life care, undertake assessments and care plan reviews, and work closely with families, GPs and district nurses to deliver high-quality care in patients’ homes.

You will provide leadership to nursing and care teams, mentor staff, and contribute to governance and quality improvement.
