Job Description:
West Suffolk Clinical Commissioning Group is seeking a Frailty Practitioner to deliver autonomous clinical assessments, diagnosis, and treatment for frail and vulnerable patients in their own homes and local settings.
The post holder will work within SS9 South PCN, collaborating with GP practices, community teams, and social care to prevent hospital admissions and support timely discharge where appropriate.
